YOU WILL BE CHARGED FOR THE INSURANCE AFTER JULY 1, 2009 IF YOU DO NOT OPT OUT.

About the REMINDERS sent back with your dues receipt. The life insurance takes effect July 1, 2009, therefore our
computer systems is already charging the 5.00 per month premium. Because of this many members are coming up
short for the third and fourth quarters. I am enclosing a reminder with your paid dues receipt and writing the amount
of credit on your receipt. This would be the amout of credit you have toward the next month or end of the quarter.
Again I can't stress the importance of sending in the opt-out form if you are not taking the insurance because you are
being charged for it and your dues will be short. You can douwload the form HERE.
